We kindly request you complete a questionnaire (download link below) prior to your appointment which helps you to get more out of you first consultation. Grace will then arrange assessment by a Dietition followed by an appointment with Mr Andrew Harris. The consultation fee covers both the consultation with a Dietition and Mr Andrew Harris.
